Enrolling Your Child

ENROLLING YOUR CHILD

You may come by the Woodland Office for enrollment forms. You may also download and print enrollment forms and return the forms to Woodland along with the documentation listed below.  To download copies of enrollment forms, click here:  ENGLISH  or  SPANISH.  

THE FOLLOWING DOCUMENTATION IS NEEDED FOR ENROLLMENT:

  • Birth Certificate
  • Social Security Card
  • Shot Records
  • Parent or Guardian ID (Driver’s License, Passport, or State ID)
  • Proof of Residence – one of the following:
    • Utility Bill (Water, Gas, or Electric)
    • Proof of owning home
    • Lease agreement
  • Withdrawal form from previous school district
  • If not custodial or legal guardian, you will need to complete a Voluntary Caregiver Form

Note:  If applicable, please provide any documentation for additional services such as Special Education, 504, Gifted and Talented, or Dyslexia.
